Student/Parent/Guardian Online Safety Agreement

1. I will not give out personal information such as my address, telephone number, parent's work address/telephone number, or the name and location of my school without my parents' permission.
2. I will tell my my teacher or parents right away if I come across any information that makes me feel uncomfortable.
3. I will not respond to any messages that are mean or in any way make me feel uncomfortable.  It is not my fault if I get a message like that.  If I do, I will tell my teacher or parents right away so that they can contact the service provider
4. I will respect all school property.  I will not attempt to harm, change or destroy any software or hardware. 
5. My parents and I will read and discuss the District online policy.
6. Misuse of computer network by breaking any of of the rules listed in this contract will cause the Osborn School District to take disciplinary actions in accordance with district policy.

Consequences for infraction of the online  rules: 

  1.  1st offense will result in student being sent to the responsibility room.

  2.  2nd offense will result in the student being sent to the responsibility room and the parents will be called.

  3.  3rd offense will result in the student being sent to the responsibility room and computer privileges will be suspended.

I understand that it is impossible for the School and District to restrict access to all controversial or offensive materials and that all teachers and staff will provide reasonable supervision to prevent access to inappropriate materials.  I will not hold the teacher, staff or district responsible for materials acquired by my child's use of  Electronic Information Services.

I agree that my child will abide by the Osborn School District Acceptable Use Policy and rules on appropriate use of Electronic Information Services.  I hereby give my permission to have my child use Electronic Information Services ad certify that my child has agreed to abide by the terms and conditions of this agreement.
