6/26/00 Lila called approx 3:30 explaining that Alex, Calvin's assistant, was refusing to work with him if our representitive, Calley Fauth would be present. Lila apologized that this was the outcome but explained she couldn't control Alex's feeling on this matter. I told Lila that I understood her (Lila's) problem here but that I would still like a written explination from her. I conceeded that having Alex walk out would not be in Calvin's best interest so I would ask Calley not to attend the sessions with Alex even though I believed, as Calvin's parent, that Calley would add value to the situation. Lila added that Alex's 'free spirit' was hard to control so I left it at that. I forgot to ask if they would have a similar problem if Calley attended
the later session where Tracy, the new assistant, would take over. I phoned back at about 3:45 and asked this. Lila didn't think it would be a problem but asked that we define both their roles with Calvin while they were in the classroom.
6/21/00 Requested prior written notice yesterday, 6-21-2000, from Lila on the phone at 4:40pm via Tim if Calley's presence is not acceptable at Kindercare. (why would they deny an independent observer with parental approval at a private preschool).
6/22/00 Tim spoke with Alex, the EI aide today at 9am. She was very adimate about not working in the conditions where Calley would view her work and she would have to "teach" or be observed in general. She said she was not notified of Calley.
Tim pointed out that Calley could be used as a resource for Alex and the teacher (which is different for summer).
Alex would consider it and speak with Lila.
4/7/00 Spoke with Mary Broadhurst. We will retain her for an evaluation of Calvin's case.
10/2/99 2nd consult from Robbin Sobotka-Soles. Need to finish Verbal Imitation sheet (backwards).

9/17/99 Spoke with Kim Gillis, Autism Specialist
I finally spoke with Kim. Here is what I gleamed from our almost 1 hour conversation:

1. She does not really have a good idea of what extent she can help us. She is new in the position and doesn't know what latitude her boss, Lila Kuhkendal, will give her as far as consulting with parents. She was willing to find out if she could meet with Tim, I, and Calvin in our home sometime in the next two weeks to help us with discrete trial. She did not think she could get permission to talk with Shauna and I (and Tim and Bill, of course!) together.

2. She sounded extremely willing & pretty knowledgeable with some experience. She is a speech therapist (I think she said certified in Mass), worked in Boston in what she called "and excellent autism program for little kids". That is where she gained her experience with autism and ABA. She believes in ABA/Discrete Trial. She missed a very important planning committee meeting day for the autism classroom due to the death of her mother. She seemed surprised at what I had told her about Cascade Regional. Essentially I told her that I have given up on her position being helpful because of the turnover, lack of direct ABA training for parents, and the refusal to have Calvin's case reassigned to another Autism Specialist right after we found out Calvin was autistic with CDRC and Julie leaving. She also seemed surprised when I told her I made Lila come to Calvin's IFSP meeting and that Lila instructed the case manager to remove "autism specialist" from his IFSP.

3. Hanna is not on her list to receive Autism services. Shauna will need to call an IEP meeting and request an evaluation plan to evaluate Hanna for autism from Cascade Regional. Kim and a speech therapist would be the ones to do it. I can help you with this if you supply a copy of ALL important paperwork Early Intervention has supplied you with (ALL evals, ALL versions of your IFSP).

Sounds like Kim has helped families in Corvallis with a home program (not sure about hiring students) but she is unsure of how much she is willing/able to help us. She might just turn out to be a good low level resource that we don't have a whole lot of access to in the least, or the answer to our prayers at best.
9/12/99 Life Shift course through Threshold (Sharone Lee)
9/14/99 Spoke with Diana about Calvin's first evaluation that essentially described Autism but was not referred for Autism until December! So between the report Dated 6/9/98 and December 98 no one referred Calvin for Autism Evaluation.
9/11/99 First ABA consult with Robbin. Lindsey, Denise, Callie, I, and Tim attended 5 hour workshop. Taught ABA principals, did general overview and testing of Calvin's current skills, and created a curriculum.
8/31/99 Met with Beth Steege in Beaverton. She is the Outreach Director for CDSO.
8/16/99 Met with Robbin in Portland. She is a private consultant. We hired her for training and long term consulting
7/10/99 Returned from 1 week vacation in Pendleton with Dad. Would not sit, follow a one step direction, or stop whining within 2 hour period (usually does not whine at all). Took 2 days for recovery until whining subsided
6/14/99 Started one on one daycare with Lindsey, OSU Teaching Grad student 7 hrs a day 5 days a week.
4/30/99 Started training in PRT from hired person.
4/28/99 IFSP meeting determined PRT 5 hours per week by aid, assistant, and speech therapist, ESY 4 hours per week, pre-school at Lincoln for 6 hours per week, assessment by OT, future OT services, 1 hour per week of speech, aid to do some of PRT. Consistant 2 word sentences not always with "No" or "Yeah". Emerging 3 word sentences occassionally spontaneously "Dadda no milk" "No tickle please". Increased eye contact.

3/30/99 Start weekly class Live and Learn with your Child with Special Needs with Sara Gelser and Geri Aman
3/21/99 Started First Step course through Threshhold, a monthly support and informational meeting for parents with autistic children
3/15/99 Started on Super NuThera, Liquid / Folic Acid and DMG pills. Vit A 5000iu, Vit C 500mg, Vit B1 15mg, Vit B2 15mg, Vit B6 500mg, Vit B12 10mcg, Pantothenic Acid 20mg, Vit E 30iu, Folic Acid 400mcg, Magnesium 120mg

DMG 125mg, Folic Acid 400mcg. Consistantly using 2 word sentences "No Momma", "Yeah juice" at least 40% spontaneously. Started at least 2 hours one on one at home with parents
3/12/99 CDRC. Evaluation: Autism Evaluators: Laurel Robertson, MA, Cynithia Hartun, PhD, Darryn Sikora, PhD, Patricia Garlinger, MD, Kathyrn De Armond, MS, CCC, Rodney Pelson, PhD (results given to us immediately)
1/12/99 Started on B6, Magnesium, and DMG. Starting to say Momma/Dadda in a meaningful way. More pointing and taking parents by hand to what he wants
12/17/98 IFSP to tell us of Cascade Regional Results
12/14/98 IFSP to discuss possible autism. Calvin's speech has made minimal progress. No more words, no more eye contact, still cries to get objects/food instead of pointing
11/4/98 IFSP
6/9/98 Benton County Early Intervention/Early Childhood Special Education Assessment Information. Evaluators: Pat Hill, Mariana Jones, and Barbara Beardsley. Recommendations: Toddler Group 1 time per week for 1 hour, summer services for maintenance
6/2/98 IFSP meeting at Kindercare. Benton County EI became involved due to referral from parents. Teacher Jan started whole process by indicating Calvin's language is not age appropriate. She informed us of Benton County Early Intervention and the fact that it is free. Calvin has vocabulary of about 50+ words, rarely has spontaneous 1 word sentences.
